Community Chest Po Tat CareAge Nurse Station

The project aims at filling in service gap in the local public health system by offering on-site spontaneous nursing care service to the elderly residing in Po Tat Estate.  The services include health screening for early detection and intervention, one-on-one case management and medical consultation to frail elderly, health education for the general public, as well as volunteer service and support for caregivers.  Further, the project networks with other professional medical teams to take care of functional competency of the elderly - eye care, dental care, hearing ability.  All services are free of charges to cater to the financial needs of the underprivileged elderly.
