By Your Side Project : Support to Elders Suffering from Stroke or Dementia and their Caregivers

By Your Side Project aims to support community residing elders who have been discharged from formal care programmes due to stroke or dementia symptoms and their caregivers.  The services include personalised home-based rehabilitation programmes supervised by physiotherapists, occupational therapists, and speech therapists.  Regular reassessments will be conducted to monitor the rehabilitation progress of the participants and provide appropriate guidance to family caregivers.  These therapists will also provide training and guidance to family caregivers.  Mutual help groups will be conducted for participating elders and their family caregivers to promote the spirit of mutual support.
