Sai Kung Wellness Connection

'Sai Kung Wellness Connection' introduces a tailored rural-specific service model that promotes the concept of "Ageing in place", effectively enhancing the well-being and health status of elderly individuals through a preemptive and minimally invasive healthcare approach.  By improving accessibility to primary healthcare services and using social prescription to enhance health outcomes, the project can improve the overall resilience and sustainability of the community's healthcare system at a lower cost.  Additionally, this project places a strong emphasis on the utilisation of "Evidence-Based Practice" to contribute valuable insights to develop a pilot service model aimed at promoting a tailored service approach for rural settings.
